*The Valley locations* *Position Summary* The primary purpose of your position is to maintain clinical processes and systems, delegate certain nursing tasks, which may include medication administration, to unlicensed assistive personnel (UAP), and providing training and education in Assisted Living communities throughout your assigned Cluster.
You will work with the communities to assure that the highest degree of quality care can be always provided to our residents.
*Essential Functions and Responsibilities* · Ensure the CAPLICO Core Values and Code of Conduct are adhered to at all times.
· Ensure compliance with Resident Rights and HIPAA policies at all times.
· Collaborates with the Executive Director, Community Liaison, and Wellness team to assess residents’ needs and to place them appropriately within the continuum of care.
· Ensures completion of resident assessments and service/care plans in accordance with community policies and state regulations, reviews them for appropriateness, ensures that service/care plans are available to appropriate staff.
Ensures that residents receive the services/care specified in their plans.
· Identifies opportunities for improvement within community Wellness Departments through frequent visits and communicates information to the Executive Director and Wellness Leader.
· Observe and teach infection control, safety, and resident rights policies and processes.
· Responsible for planning, scheduling and conducting training and education specific to the responsibilities of the staff.
· Maintain current education records of classes and in-services conducted, to include attendance and performance of each attendee.
· Continually assesses training programs to validate the approved programs are being implemented.
· Assume the authority, responsibility and accountability of RN Delegator/LPN · Understand all applicable laws, rules and regulations that affect the delegation process and ensure compliance within the community.
· Complies with the Five Rights of Delegation to be sure that any delegation is: the right task, right circumstances, right person, with the right directions and communication, and under the right supervision and evaluation.
· Determine when and what to delegate based on the community setting, the residents’ needs and condition, the state/jurisdiction’s provisions for delegation, and the community policies and procedures regarding delegating a specific responsibility.
· Ensure UAPs have the appropriate knowledge, skills, and competency to accept any delegation.
· Evaluate competency of unlicensed staff for each delegated task on a continuous basis.
· Continuously evaluates the delegation process to ensure compliance.
· Compiles, analyzes, trends and reports quality data in the following areas: resident care, safety, risk management, infection control, outcomes, and resident satisfaction.
· Participates in strategic planning and the development of market initiatives by providing input from clinical and leadership perspectives.
· Participates in designated Community, Cluster, Market and Resource meetings.
· Performs general nursing duties as necessary.
· Maintains current nursing license according to state Board of Nursing requirements/LPN requirements · Other related duties and responsibilities that may become necessary to ensure that the needs of the communities and market are met.
*Supervisory Responsibility* · Exercises administrative and functional supervision over all unlicensed assistive personnel to whom tasks have been delegated.
*Work Environment* · Works in office area(s) as well as throughout the communities.
· Frequent travel between assigned communities (50% or more if needed).
(Parkside Senior Living in Neenah, Lakepoint Villa in Oshkosh, Maple Meadows in Fond du lac, and Scandinavian Court in Denmark) · Moves intermittently during working hours.
· Is subject to frequent interruptions.
· Is involved with residents, personnel, visitors, government agencies/personnel, etc.
, under all conditions and circumstances.
· Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
· Is involved in community/civic health matters/projects as appropriate.
· Is subject to injury from falls, odors, etc.
, throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
· Is subject to exposure to infectious waste, diseases, conditions, etc.
, including TB and the AIDS and Hepatitis viruses.
· May be subject to the handling of and exposure to hazardous chemicals.
· Will have monthly market meetings to attend.
· Will have care meetings or conferences to attend to for residents as needed.
*Physical Demands and Sensory Requirements* (With or Without the Aid of Mechanical Devices) · Must be able to move frequently throughout the workday.
· Must be able to cope with the mental and emotional stress of the position.
· Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of this position can be fully met (e.
g.
: accurately read measurements on resident related equipment such as thermometers, monitors, gauges).
· Must be able to function independently, have personal integrity, flexibility and the ability to work effectively with residents, employees, visitors and support agencies.
· Must be in a good general health and demonstrate emotional stability.
· Must be able to relate and work with the disabled, ill, elderly, emotionally upset and at times, hostile people within the facility.
· Must be able to lift, push, pull and move a minimum of 50 pounds.
· Must be able to assist with the evacuation of residents.
· Must be able to pass resident medications if needed or directed to.
· Must be able to assist staff and work on the floor which could be any of three shifts *Specific Requirements* · Ability to speak and understand the English language.
· Possess the ability to make independent decisions when circumstances warrant such action.
· Possess the ability to deal tactfully with personnel, residents, visitors, and the general public based on whatever maturity level they are currently functioning.
· Ability to be calm and level-headed in emergencies.
· Well groomed, professional and possess ability to work harmoniously with other personnel.
· Ability to seek out new methods and principles and be willing to incorporate them into existing resident care practices.
· Follows written and oral instructions.
· Knowledgeable in the use of computers, e-mail, spreadsheets and document word processing.
· Maintain patience, tact, cheerful disposition and enthusiasm, as well as ability to handle residents, staff, and visitors, based on whatever maturity level they are currently functioning.
*Education and Experience Requirements* · Unencumbered RN license or LPN license One year experience in a similar role or equivalent education and training.
Job Type: Full-time Pay: $70,000.
00 - $90,000.
